As a current stockholder, you will need to write the board of directors, saying who else you would like added to the stock.  The person to be added needs to submit a request in writing, as well.  Both documents need to be notarized, though they need not be in any particular form.  A current Credit Report Authorization will need to be signed for all new persons being added.  A fee of $12.00 will be charged for processing.
